The first time you reach the end of Act 3, the Scion will be encountered shortly before the final boss fight. After clicking on her cage, she will appear in the character selection screen.

Killing Dominus will also unlock her if you missed the cage. Dominus, High Templar is the final boss of act 3, located in The Upper Sceptre of God, on the Tower Rooftop (after climbing 3 or 4 staircases in the Upper Sceptre of God).
