Ventar the Unholy can be found in the Throne of Destruction and is summoned as the fourth boss wave of five in the pre-encounter with Baal.

Quests: Ventar and his entire pack must be killed or moved out of Baal’s sight range (to the end of the throneroom) before Baal will summon up the next bunch of monsters. All the packs must be destroyed to continue on your way to completing the Eve of Destruction quest.
Comments: Ventar can spawn with MSLE on Hell, but after v1.10 that combination of modifiers is no longer dangerous enough to take precautions to avoid. Ventar represents Act Four, in Baal’s sequential run of Act-themed monster packs.
